Geographic Scope: Minnesota (Twin Cities area)

Focus Areas: Three rotating annual priorities:

1. Elderly Programs (cycle year 1) - Services to help seniors live independently
   • Home-delivered meals
   • Transportation
   • In-home medical assistance
   • Home safety modifications
   • Senior center programs
2. Youth Programs (cycle year 2)
3. Pregnancy and Single Parent Services (cycle year 3)

Eligibility:

• Nonprofit organizations serving vulnerable populations
• Past recipients of CCF support preferred
• High-quality program track record

Funding Details:

• Award range: $8,000-$12,000 per grant
• Example year (2016 elderly focus): $85,000 distributed to 8 organizations
• Applications currently solicited by invitation only due to limited funding

What They Fund:

• General operating support
• Program-specific initiatives
• Services supporting vulnerable elderly, youth, and families in need

Rotation Schedule:

• 2016: Elderly Programs
• 2017: Young Mothers and Fathers in Need
• 2018 (projected): Youth Programs

Additional Information:
Designed to help the most needy in the community. Due to limited funding, CCF typically only funds about one-third of qualified applications received.
