We are supportive of collaborative approaches to addressing community health issues, and we welcome proposals developed by partnerships and coalitions with clearly articulated goals and strategies. Each applicant may apply for funds up to $5,000 each grant funding cycle. Priority areas Funding decisions are based on how well proposals align with Sky Lakes’ priorities. Your proposal will need to target and have a meaningful impact on at least one of the priority areas listed below. Multiple connections between priority areas are encouraged. Health, wellness, and safety activities and programs STEM&M (Science, Technology, Engineering, Math and Medicine)-related education events and programs Specific economic development programs that are consistent with the Sky Lakes missionCommunity Health Implementation Plan (CHIP) priority areas: Equity; Physical Activity; Food Insecurity; Health Promotion: Access to Services; Substance Abuse; Mental Health The Community Health Improvement Plan is a long-term, systematic effort to address health issues and concerns, and the factors that influence them. The CHIP is used by healthcare agencies, in collaboration with community partners, to establish priorities and to coordinate activities and resources to improve the health and well-being of community members, and the overall health status of the community.Eligibility
