Eligibility

• Families with children from birth to age 8
• Household income under $150,000
• Child must meet accepted diagnoses covered by The Shooting Star Foundation
• Child's therapy provider must be pre-approved by The Shooting Star Foundation
• For families whose children use therapy centers outside the primary approved partners (Sunny Days Therapy, St. David's Center, Fraser)

Program Details

• The Foundation acts as a third-party pay source directly to the therapy provider
• Funds are awarded to the therapy partner to be applied to client accounts for billed services
• Applicants should submit information through the form on the How To Apply page if their child meets scholarship criteria and is approved to use another therapy provider outside of the standard approved partners
• The Foundation will attempt to connect families with other therapy centers
