Eligibility

• Families with children from birth to age 8
• Household income under $150,000
• Child must meet accepted diagnoses covered by The Shooting Star Foundation
• Must be approved for services by Fraser (approved partner)
• Families with barriers needing early interventional therapies

Program Details

• Partnership between Fraser and The Shooting Star Foundation
• Provides needed services and financial assistance to those with barriers
• The Foundation acts as a third-party pay source directly to the therapy provider
• Funds are awarded to the therapy partner to be applied to client accounts for billed services
• Applicants must be pre-approved for services by Fraser before beginning the scholarship application

How to Apply

2. Begin your scholarship application after receiving service approval
3. Click "Apply Here" on the How To Apply page to submit your application
4. Complete the scholarship application form
