5. Narrative Content:

- Description of the specific project or program requesting funding
- Project/program budget and timeline
- Details of any fundraising being conducted to support the project/program

6. Grant Request:
- Dollar amount requested (maximum $2,000 per calendar year)
- Confirmation agreement to complete impact form if awarded (YES/NO)
