2. Submit by Deadline: January 30th, 2026 (for 2026 funding)
3. Required Materials/Fields:
   • Organization name
   • Organization address
   • Program director name and contact information (phone and email)
   • Organization website (or N/A if none)
   • Names of board of directors/church board members
   • Date organization was established
   • IRS determination letter (upload required)
   • Organization's mission statement
   • Project title
   • Amount requested
   • Concise statement of grant request (describing how funds will be used)
   • Description of how the project will be evaluated (include qualitative and quantitative measures)
   • Proposed budget for the project
   • Quotes and financial information related to the project
   • Explanation of how project fits into Community Health Needs Assessment
   • List of coalitions and community collaborations
   • Other financial resources applied for or planned
   • Plans for program sustainability if funding is not received or if future funding is not available
   • Name of person applying
   • Signature (type name to sign)
   • Position within organization
4. Additional Submission Options:
