Two-Stage Application Process

#### Stage 1: Letter of Inquiry (LOI)
Submit the following information:

1. Organization Mission Statement
2. Proposal Narrative - including:
- Executive summary
- Statement of the problem
- Total project budget
- Project evaluation methodology
3. Intended Impact Description - brief description of intended impact from funds being raised
4. Type of Funding Being Sought - select from:
- General support
- Scholarships
- Program/project
- Capacity building
- Research
- Challenge or matching
- Seed money/start-up
- Emergency
5. Community Information - state which community your organization operates in (must be in RJAFF's service areas)
6. Eligibility Confirmation - clarify that organization is a 501(c)(3)

LOI Responses: Reviewed on rolling basis

#### Stage 2: Grant Application (if LOI is accepted)
Submit the following if invited:

1. One-page Cover Letter (required; doc, docx, or pdf format; max 4 MB, or Google Doc link)
2. Organization Information:
- Legal name (as on IRS exemption letter)
- Physical address
- Mailing address (if different)
3. Contact Information:
- Contact person name and title
- Email address
- Phone number
- Fax number
- Website
4. Funding Request:
- Amount of money requested
- Total cost of project
5. Project Description (no more than 3 sentences):
- How grant funds will be used
6. Detailed Project Description - minimum requirements:
- Number of clients to be served
- Community need met by proposal
- Timetable and/or work plan
7. Project Funding Plan:
- If project costs exceed request amount, explain how balance will be funded
- Show any in-kind support, other grants applied for, funds already received
8. Organization Information:
- Mission statement
- Primary goals
- Recent major accomplishments
9. Project Leadership:
- List of staff leading project with qualifications
10. Governance:
- Board of Directors list with addresses and occupations
11. Financial Information:
- Organizational budgets
- Basic financials (Profit and Loss and Balance Sheet)
- Most recent audit available
