About the Funder

The AIDS Funding Collaborative has been the leading public/private partnership focused on the HIV effort in Cuyahoga County since 1994. The AFC has mobilized nearly $13 million for the HIV response in Cuyahoga County. Funding partners include: The Alcohol, Drug Addiction, and Mental Health Services (ADAMHS) Board of Cuyahoga County, The City of Cleveland, The Cleveland Foundation, Cuyahoga County, The George Gund Foundation, and Mt. Sinai Health Foundation. The Center for Community Solutions is the fiscal home of the AFC.

Mission

To strengthen the community's response to HIV/AIDS as a public-private partnership providing coordination, leadership, advocacy, and funding in Cuyahoga County. The work advances timely and equitable access to quality prevention and care—improving quality of life for those at-risk or living with HIV/AIDS in Cuyahoga County.

Application Process

For 2025 Responsive Grants:

1. Letter of Intent (LOI) Phase:
- Download the 2025 AFC Letter of Intent Form
- Submit LOI to indicate intent to apply

2. Full Application Phase:
- Invited organizations submit full proposals in response to the AFC's Request for Applications (RFA)
- Complete Budget Worksheet
- Submit all required documentation
