Black River Education and Wellness Foundation - Grant Program

Foundation Overview:
Black River Education and Wellness Foundation is a tax-exempt 501(c)(3) charitable foundation organized in 1985. Originally chartered to support Elyria Memorial Hospital operations, the foundation broadened its mission in 2014 following the acquisition of what is now UH Elyria Medical Center by University Hospitals of Cleveland.

Geographic Scope: Lorain County, Ohio

Increasing access to care

Education and wellness initiatives

Funding Focus:
Only requests that demonstrate direct relationship to:

Health issues of a community or group within a community, OR

Improved health, quality of life, or access to care within Lorain County communities

Requests are entertained and considered on a rolling basis throughout the calendar year. Decisions on merit and disbursement timing are entirely at the discretion of the Foundation Board.

Submit as much supporting information as possible

Key requirement: Clearly describe the relationship of the proposed or continuing project/program to improved health or improved access to care within the community

If equipment purchase is included: Append all proposed equipment information to the request application

Step 2: Sign and Date

Sign and date the Request for Grant Funding

Ensure signatory has authority to act on behalf of the organization

Step 3: Submit

Mail the completed Request for Grant Funding along with all supporting documentation to:
