Rehabilitation Management and Services for the New York State Veterans Home at Montrose Through this Request for Proposals (RFP), the New York State Department of Health (the Department) is seeking competitive proposals from qualified bidders to provide the management of and therapy staff for the Physical Therapy Rehabilitation Department at the New York State Veterans Home at Montrose (NYSVH), as further detailed in Section 4.0 (Scope of Work). It is the Department’s intent to award one (1) contract from this procurement. Introductory Background The NYS Veterans Home at Montrose (NYSVH) is a 252-bed long- term care skilled nursing and rehabilitation facility owned and operated by the New York State Department of Health. NYSVH is a state-of-the art facility serving qualified Veterans of the United Stated Armed Services and their qualified dependents. The facility is located in Northern Westchester County, roughly 35 miles north of New York City on the grounds of the U.S. Department of Veterans Affairs (VA) Franklin D. Roosevelt VA Medical Center. The facility is organized into six (6) care units, each accommodating 42 residents, with two units specifically designated for memory care.
