Application Process

1. Access Application: Online form available at uweca.org/hopeapplication/

2. Provide Personal Information:
- Full name and maiden name/other names used
- Last 4 digits of Social Security Number
- Current address and length of residency
- Race and marital status

3. Household Information:
- List all household members with:
- Name, relationship, and date of birth
- Last 4 digits of Social Security Number
- Gender and race
- Source of income and amount
- Include food stamp receipt status and amount
- Indicate if applying for child support
- Veteran status for any household members

4. Employment Information:
- Current employer name and job title
- Hours per week and length of employment
- Spouse/partner employment status (if applicable)
- If unemployed, length of unemployment

5. Financial Assistance Request:
- Describe what assistance is being requested
- Amount of assistance needed
- Amount applicant can contribute
- Explanation of hardship/what happened

6. Monthly Expense Details:
- Rent or mortgage
- Power bill
- Gas bill
- Water bill
- Car payment
- Insurance
- Prescription medications
- Credit cards
- Other monthly expenses

7. Contact Information:
- Email address (required)
- Phone number (required)
- Best time to call (time selection required)
- Preferred contact method (email or phone)

8. Documentation:
- Upload copy of utility bill being requested assistance with (max 2 GB, up to 4 files)
- Upload photo of Driver's License (max 2 GB)

9. Consent and Authorization:
- Sign consent form authorizing access to Alabama Power account information
- Confirm accuracy of information
- Acknowledge understanding of authorization and revocation rights
- If not account holder, confirm authorization as representative

10. Submit: Complete online form submission
