Racial Equity in Postpartum Care Challenge - Phase 1

CyncHealth, Collective Medical (a PointClickCare company), and Innsena won the Department of Health and Human Services' Racial Equity in Postpartum Care Challenge for their innovative postpartum care program in Nebraska. The award recognizes collaborative work to improve postpartum care for at-risk mothers.

Focus Areas

• Target Population: Black, African American, American Indian and Alaska Native parents
• Program Focus: High-risk conditions during pregnancy and postpartum
• Goal: Significantly reduce maternal morbidity and mortality
• Beneficiaries: Medicaid and Children's Health Insurance Program (CHIP) participants

• Initial work began in Omaha, Nebraska
• Expansion expected statewide as part of Phase 2
• Program supports healthy outcomes throughout pregnancy and postpartum journey
• Healthcare teams identify at-risk mothers and infants
• Improved care coordination before, during, and after delivery

• Problem Addressed: Nebraska ranks 19th nationwide in maternal morbidity
• Racial Disparity: In Douglas County, Black infant mortality rate is 14.5 per 1,000 births (more than double the rate for White and Hispanic infants)
