Philip D. Levesque Memorial Community Award

Geographic Scope: Gifford's service area in the White River Valley, Vermont

Focus Areas: Arts, health, community development, education, or the environment

Background: Levesque, Gifford's beloved president and chief executive officer from 1973-1994. The award recognizes Levesque's personal commitment to community service and improving the White River Valley area. First awarded in 1995.

Eligibility:

• Nonprofit community organizations
• Must be located in Gifford's service area
• Must be involved in arts, health, community development, education, or the environment
• Must have 501(c)(3) certification

Selection Process:

• Committee of hospital staff and Levesque's family reviews applications
• Winner announced at Gifford's Annual Meeting in March

Application Process

Submission Deadline: February 12 (annually)

2. Estimated budget and timeline for completion
3. Expected results and benefits
4. Copy of organization's 501(c)(3) certificate
