Program Description:
The Patient Community Liaison Microgrant was created to help patient ambassadors and community leaders lead grassroots and community efforts to advance awareness, treatment, and prevention of vascular diseases, particularly Peripheral Artery Disease (PAD).

• Community outreach and engagement
• Amputation prevention through early intervention
• Health disparities in vascular disease diagnosis and treatment

• Community leaders with lived experience of vascular disease

• Patient-led initiatives

Project Examples

Kim Campbell, RN (2023 recipient): Community education project in Atlanta and Greenville, Mississippi focused on PAD awareness and amputation prevention
