Patient Assistance Fund

The Patient Assistance Fund is dedicated to supporting families with financial support due to complications related to congenital heart disease (CHD).

Funding Purpose

Financial assistance may include, but is not limited to:

• Travel assistance related to CHD medical care
• Medical bills related to CHD and not covered by insurance
• CHD specialized equipment not covered by insurance

Eligibility

• Families affected by congenital heart disease complications
• Must submit corresponding receipts alongside application

Award Amount

Up to $1,500 per quarter for expenses related to CHD complications
