Application Process

1. Contact your local Lions Club to inform them of your wish to participate in the contest
2. Download and complete required forms:
- Common Contest Application Form
- Medical Power of Attorney Form
- Code of Conduct Agreement
- Chaperone Guidelines (if providing a chaperone)
3. Fill out forms in Adobe Reader
4. Submit according to contest rules by Monday, April 20, 2026 (postmark deadline)
5. Direct questions to Contest Coordinator: Lion Gloria Mathur

Claiming Your Scholarship

1. Submit proof of registration from a university, college, junior college, or vocational school
2. Complete Scholarship Request Form
3. Include supplemental information needed by the institution (scholarship forms, etc.)
4. Send request to:

5. Timeline: Scholarship awards must be collected no later than two (2) years following recipient's graduation from High School

Note: Payment will be made directly to the designated school.
