How to Apply

1. Complete the Organizational Grant application form
2. Provide organization information:
- Organization name
- Full address (street, city, state, ZIP)
- Main phone number
- Cell phone number (if different)
- Email address

3. Provide organizational contact information:
- Contact name (first and last)
- Phone number
- Cell phone number (if different)
- Email address

4. Answer required questions:
- Brief description of the goals and objectives of the proposed organization
- Confirmation of 501(c)(3) status (Yes/No)
- Description of regular sources of income
- Specific description of resources being requested
- Detailed description of the proposed project for which you desire a grant

5. Submit completed form to Mother's Grace for review
