Organizational Grant

Charitable Purpose: Financial assistance through a grant to mothers who have endured a crisis or tragedy and thereafter have created an organization with 501(c)(3) status to help others suffering from a similar crisis.

Eligibility:

• Applicant must be a mother who has endured a crisis or tragedy
• Must have created a 501(c)(3) nonprofit organization
• Organization must serve others suffering from similar crisis
• Organization must have 501(c)(3) status

Geographic Scope: Arizona and potentially beyond (application includes all US states)

Funding Use: Grants to support implementation of described projects and organizational operations

Application Process: Complete the Organizational Grant application form

Note: Mother's Grace considers each application individually and determines assistance in its sole discretion based on consistency with organizational mission.

How to Apply

1. Complete the Organizational Grant application form
2. Provide organization information:
   • Organization name
   • Full address (street, city, state, ZIP)
   • Main phone number
   • Cell phone number (if different)
   • Email address
3. Provide organizational contact information:
   • Contact name (first and last)
   • Phone number
   • Cell phone number (if different)
   • Email address
4. Answer required questions:
   • Brief description of the goals and objectives of the proposed organization
   • Confirmation of 501(c)(3) status (Yes/No)
   • Description of regular sources of income
   • Specific description of resources being requested
   • Detailed description of the proposed project for which you desire a grant
5. Submit completed form to Mother's Grace for review
