Our Vision We seek optimal health and well-being for all, promoting vibrant communities. Health First Foundation envisions a future where all individuals achieve optimal sustainable health and well-being (mind, body, spirit) for improved quality of life in vibrant communities. We strive to eliminate disease through prevention strategies and mitigate disease through early detection, treatment, and recovery. We strive to empower individuals to achieve health and well-being through education, healthy lifestyle choices, improved behavioral health, mitigation of chronic conditions, and access to health care. We promote vibrant communities that foster positive social determinants of health, including access to education, healthy neighborhoods, holistic social and cultural environments, quality health care, and economic stability. Our Mission We advance community health and well-being through leadership, partnerships, and. innovation. Health First Foundation provides leadership, forges partnerships, and supports innovation to advance health and well-being across northern Arizona’s diverse population and geography. We inspire donors to support innovative community health initiatives of nonprofit organizations, hospitals, and medical centers. Our collaborations and investment alliances strive to bring health and well-being to the greatest number of people possible with maximum impact. We evaluate community health needs assessments to determine priority needs and use quantitative measurements to assess the sustainable impact Health First funded programs have on health and well-being for all and promoting vibrant communities. Community Health Grant Funding We seek to fund projects that involve collaborations and partnerships with other organizations. We prefer programs/projects of significant magnitude and impact with funding requests of $50,000 or more. Innovative projects requesting less than $50,000 may be considered.

Eligibility

The program/project must support Health First Foundation's mission and vision.Organizations are an Arizona 501(c)(3), government agency, school, university, municipality, or tribal entity.Religious organizations may apply for funding of non-sectarian initiatives.Organizations without 501(c)(3) status must use a fiscal agent to apply on their behalfOrganizations must be in good standing with the IRS at the time of application reviewOrganizations serve populations in one or more of the five counties within northern Arizona: Apache County, Coconino County, Mohave County, Navajo County, Yavapai CountyOrganizations must maintain an active non-discrimination policy.One letter of intent per each qualified organization will be accepted.

Ineligibility

The Foundation will not fund programs we previously funded.What funding requests are not allowed? The Foundation will not fund expenses for conferences, sponsorships, fundraising events, or brick and mortar capital projects
