Project Purpose:
Support infrastructures delivering home visiting services to improve asthma management in children with poorly controlled asthma.

Target Population:

• Medicaid and CHIP-enrolled children (ages 2-17)
• Children with poorly controlled asthma

Service Model:

• Training of clinical and Community Health Workers (CHWs)
• Cost-effective, high-quality, culturally-competent asthma home visiting services
• CHWs supervised by nurse or certified asthma educator
• Three to four home visits per child (1,145 children served)
• Education on environmental asthma triggers
• Delivery of supplies to reduce environmental triggers

Outcomes Achieved:

• Substantial decrease in hospital admissions
• Reduction in urgent care visits
• Decreased missed school days due to asthma
• Decreased missed work days due to asthma

Current Status:
CMMI funding completed; provider sites continue intervention; collaborative continues promoting sustainable financing.
