Project Purpose:
Support infrastructures delivering home visiting services to improve asthma management in children with poorly controlled asthma.

Target Population:

Medicaid and CHIP-enrolled children (ages 2-17)

Children with poorly controlled asthma

Service Model:

Training of clinical and Community Health Workers (CHWs)

Cost-effective, high-quality, culturally-competent asthma home visiting services

CHWs supervised by nurse or certified asthma educator

Three to four home visits per child (1,145 children served)

Education on environmental asthma triggers

Delivery of supplies to reduce environmental triggers

Outcomes Achieved:

Substantial decrease in hospital admissions

Reduction in urgent care visits

Decreased missed school days due to asthma

Decreased missed work days due to asthma

Current Status:
CMMI funding completed; provider sites continue intervention; collaborative continues promoting sustainable financing.
