Application Process

Required Information:

7. Purpose of the grant
8. Implementation dates (start and end date)
9. Curriculum implementation details
10. Number of programs or sessions students will participate in
11. Number of students attending each program or session
12. Program name
13. Target audience and gender demographics
14. Total project budget
15. Amount requested from foundation

Required Materials:

Brief narrative (no more than one page) describing:

- How prevention education will be implemented
- Composition of implementation team
- Evidence-based curricula to be used

For school/school system applicants: Letter of endorsement signed by principal (school-level) or superintendent (school system-level)

For school/school system applicants: Email address of principal or superintendent for grant award notification

Certification that applicant has read and understands MMF grant guidelines and application complies with those guidelines

Acknowledgment of reporting expectations (post-program surveys and tracking program reach)

Content Requirements:

Applicants must indicate which education topics will be included:

- Understanding consent
- Establishing and respecting boundaries
- Conflict resolution
- Bystander intervention training
- Recognizing unhealthy relationships

Identify other campus organizations included in outreach efforts

Provide detailed budget breakdown
