Geographic Scope: Ottawa County, Michigan

Focus Areas:

• Primary Focus: Mental health, access to healthcare, and promoting healthy behaviors around nutrition and exercise
• Target Populations: Youth, seniors, and other high-need populations

Funding Priorities:

• Programming that is new or expanded to serve larger geographic areas
• Impact focused on youth, seniors, or other high-need populations
• Collaborative partnerships across multiple organizations
• Nonprofits with high organizational and financial capacity with successful program track records

Eligibility Criteria:

• Based on 2015 Ottawa County Community Health Improvement Plan (CHIP) priorities
• Programs addressing identified critical health needs

Total Funding Allocated:

• $330,000 from MHEF
• $80,000 from GHACF
• $80,000 from CFHZ
• Total: $490,000

• TCM Counseling - $70,000
• Love INC of the Tri-Cities - $75,000
• Ottawa County Food Policy Council - $100,000
