• Access to care projects

Award Details:

• Award range: $5,000 - $50,000 on average
• Award duration: One to five years depending on scope and length of program/activity request
• Number of annual awards varies

Eligibility Requirements:

• Applicants and proposed program must be located within the state of Michigan
• Only one application will be considered per grant cycle per applicant

Reporting Requirements:

• Successful applicants must submit a final report describing program outcomes, including successes and challenges
• Report deadline: April 17 of the following year
