Overview: MAP International's Custom Pack allows organizations to select medicines and health supplies for medical mission trips from an inventory of frequently-used products in communities served by medical mission teams.

    How to Apply

    Application Process for MAP Custom Pack

    Step-by-Step Process:

    1. Complete MAP Eligibility Form
    - Required document to verify medical mission trip qualifies for MAP product
    - Includes questions about yourself, your organization, and planned medical mission trip
    - Submit completed form to MAP

    2. Complete MAP Practitioner's Agreement (if ordering Rx products only)
    - Required document to verify you are qualified to order and receive Rx products
    - Must be signed by a licensed Physician or Nurse Practitioner
    - International practitioners: MAP may require additional documents to evaluate medical credentials
    - Submit completed form to MAP

    3. Submit Custom Pack Order
    - Complete after Eligibility Form and Practitioner's Agreement are submitted
    - Download or print the custom inventory shipping information form
    - Select from available inventory
    - Send back by scanning or emailing to orders@map.org

    4. Application Review
    - MAP Medical Missions Team reviews all three required documents (Eligibility Form, Practitioner's Agreement, and Custom Pack order)
    - MAP Medical Missions team will contact you with any questions about your application

    5. Approval and Next Steps
    - Once approved, MAP will contact you with next steps for processing your order
