Mammography Assistance Program (M.A.P.)

Financial assistance program for individuals seeking funding for breast screening mammograms. Provides 100 percent funding to receive this life-saving exam for early detection of cancer.

Geographic Scope: McDonough County and west central Illinois

Focus Areas:

• Breast cancer screening and early detection
• Diagnostic testing (ultrasound, sonogram, unilateral mammogram)

Eligibility:

• Family size and income-based guidelines:
  • 1 person: less than $37,470
  • 2 people: less than $50,730
  • 3 people: less than $63,990
  • 4 people: less than $77,250
  • 5 people: less than $90,510
  • 6 people: less than $103,770
  • 7 people: less than $117,030
  • 8 people: less than $130,290
  • 9 people: less than $143,550
  • 10+ people: less than $156,810
• Those without health insurance ages 40-64 may be eligible for Illinois Breast and Cervical Cancer Program (IBCCP) and/or Susan G. Komen funding through McDonough County Health Department

Additional Coverage:

• Additional diagnostic testing such as ultrasound, sonogram, and/or unilateral mammogram if ordered by physician

Application Process

1. Initial Eligibility Check:
   • Answer questions about health insurance status and birth date
   • If no health insurance and ages 40-64, contact McDonough County Health Department at (309) 837-9951 for Illinois Breast and Cervical Cancer Program (IBCCP) or Susan G. Komen funding
2. M.A.P. Application:
   • Call Patient Financial Services at (309) 836-1528 or (309) 836-1529
   • Download and complete the Financial Assistance Application
   • MDH Patient Financial Services will determine final eligibility
3. Schedule Mammogram:
   • Call Radiology at (309) 836-1550
   • Physician order not needed for screening mammogram if over 40
   • Physician order required if under age 40
