    - 5 people: less than $90,510
    - 6 people: less than $103,770
    - 7 people: less than $117,030
    - 8 people: less than $130,290
    - 9 people: less than $143,550
    - 10+ people: less than $156,810
  • Those without health insurance ages 40-64 may be eligible for Illinois Breast and Cervical Cancer Program (IBCCP) and/or Susan G. Komen funding through McDonough County Health Department

    Application Process

    1. Initial Eligibility Check:
    - Answer questions about health insurance status and birth date
    - If no health insurance and ages 40-64, contact McDonough County Health Department at (309) 837-9951 for Illinois Breast and Cervical Cancer Program (IBCCP) or Susan G. Komen funding

    2. M.A.P. Application:
    - Call Patient Financial Services at (309) 836-1528 or (309) 836-1529
    - Download and complete the Financial Assistance Application
    - MDH Patient Financial Services will determine final eligibility

    3. Schedule Mammogram:
    - Call Radiology at (309) 836-1550
    - Physician order not needed for screening mammogram if over 40
    - Physician order required if under age 40
