Geographic Scope: Austin, Texas (partnership locations: Spero Rehab and Easter Seals)

Focus Areas: Spinal cord injury recovery and rehabilitation

Purpose: Provides Occupational and Physical therapy resources to individuals who have spinal cord injuries and are uninsured or underinsured.

Must have a documented neurological deficit determined by the evaluating team and committee
Must be able to utilize partnership therapy locations in Austin, Texas
Must be willing to comply with therapy requirements
Must demonstrate excellent attendance
Must maintain open communication between physician, therapists, family, and scholarship committee
Must utilize community gym services

Program utilizes a multi-disciplinary approach with client, physician, and therapy team
Includes ancillary services such as community gyms
Therapy provided at: Spero Rehab and/or Easter Seals

Physician Completion - Have your physician complete the "Treating Physician" Section on the Application
HIPAA Authorization - Download and complete the HIPAA form
Therapist Evaluation - Have your Physical and/or Occupational Therapist perform an evaluation and complete the "Treating Therapist" section on the Application
- Therapist must work at Spero Rehab or Easter Seals
Submit Application - Submit completed application to the LSPF Scholarship Committee by either:
- Dropping it off at Spero Rehab Central, OR
- Emailing to: scholarships@lonestarparalysis.org

Physician's completion of "Treating Physician" section
Completed HIPAA form
Treating Therapist's evaluation and completion of "Treating Therapist" section (from Spero Rehab or Easter Seals provider)
