Lilly Endowment Community Scholarship Book Stipend

Funder: Community Foundation of Southern Indiana (administered through Lilly Endowment)

Geographic Scope: Clark and Floyd County, Southern Indiana

Award Amount:

Rising Sophomores and Incoming Freshmen: $900 per year

Cohorts 2010 and 2011: $800 per year

Purpose: To cover major book and equipment costs required for college attendance

Eligible Expenses:

Required textbooks and novels

Required equipment (lab goggles, nursing attire, boots for physical education classes, etc.)

Required items for courses (zip drives for specific classes, required media)

Choir uniforms/robes for credit-bearing courses

Photography supplies or film developing for photography courses

Cap and gown for graduation

Horse-back riding class equipment

Ineligible Expenses:

Basic school supplies (paper, pens, pencils, folders, notebooks, planners)

Ink cartridges

Computer supplies not specifically required by professor

GRE or other test preparation materials

Travel expenses of any kind

iPad or devices not expressly required

Uniforms for volunteer organizations or non-credit bearing courses

Miscellaneous supplies

Praxis test registration (reimbursed separately)

Special Notes for Education Majors:

PRAXIS test registration reimbursed separately

Submit registration receipt to community foundation

Student teaching supplies (index cards, markers, etc.) eligible

Reporting Requirements:

Complete Book Stipend Report Form at end of academic year

Provide receipts for all purchases

Report all expenses, even if exceeding stipend amount

Return unspent funds ($25 or more) to foundation before end of academic year

Keep unspent amounts of $25 or less for general school supplies

Failure to submit report results in ineligibility for next year's stipend

How to Apply

Lilly Endowment Book Stipend Report Process

Submission Steps:
1. Collect all receipts for books and equipment purchased
2. Complete Book Stipend Report Form (online or hard copy)
3. Provide the following information:
- Student name
- Email address
- Cohort year scholarship was awarded
- County (Clark or Floyd)

4. List all books and purchases with:
- Date of purchase
- Class name
- Book/item name
- Amount spent

5. Calculate totals:
- Subtotal of book/equipment purchases
- Add sales tax and shipping charges
- Calculate total amount spent
- Identify unspent amount

6. Upload supporting documentation:
- Photo copies of original receipts (required)
- Maximum file size: 32 MB

7. For online purchase receipts:
- Report actual amount paid, not campus price
- Include shipping and handling in reported amount

8. Submission Options:
- Complete and submit online form
