Our care model enables our teams to think and work as one, coordinating your care seamlessly, so you don’t have to — and delivering better care when it matters most. Our members have access to care from Kaiser Permanente health care professionals by phone or video 24/7. Care for members and patients focuses on their total health. Permanente Medical Group physicians, specialists, and teams of caregivers guide all care. Our medical teams are enabled by industry-leading technology and tools for health promotion, disease prevention, care delivery, and chronic disease management. Washington Region - Community Health Grant To better understand the needs of each community we serve, we conduct a Community Health Needs Assessment every 3 years. This strategic process informs our community investments and helps us make long-term, equitable, and sustainable changes. It also allows us to strengthen relationships with community organizations as we work together to improve community health. Eligibility
