Provides financial assistance to families experiencing hardship due to a child's hospitalization.

Geographic Scope

• Primary: Georgia (medical facilities in the state of Georgia)
• Secondary: Children's of Alabama, Children's at Erlanger

Eligibility Requirements

Applicants must meet ALL of the following criteria:

1. Child received treatment at a medical facility in Georgia, Children's of Alabama, or Children's at Erlanger
2. Legal resident of the United States of America
3. Child/children are 21 years of age or under
4. NOT currently in the process of filing for bankruptcy or carrying out the terms of a bankruptcy
5. If homeowner, NOT more than 2 months behind on home payment

Application Process

1. Eligibility Screening: Complete a 5-question eligibility questionnaire:
   • Question 1: Did your child receive treatment at a medical facility in Georgia, Children's of Alabama, or Children's at Erlanger?
   • Question 2: Are you a legal resident of the United States of America?
   • Question 3: Is your child/children 21 years of age or under?
   • Question 4: Are you currently in the process of filing for bankruptcy or carrying out the terms of a bankruptcy?
   • Question 5: If you own your home, are you more than 2 months behind on payment?
2. Upon Approval: If eligible, applicants receive a "START YOUR APPLICATION" button to begin the formal application
3. Submit Application: Complete and submit the full application form through the online portal
