- Renewable: Scholarship is renewable for a second year, contingent upon satisfactory academic progress and funding availability

Application Process
1. Complete online scholarship application on the foundation website
2. Submit by April 1 for consideration in the upcoming academic year (applications after this date may not be considered for the immediate upcoming academic year)
3. Awards will be communicated prior to end of May each year

- Current GPA (on 4.0 scale)
- Name of current school and school for scholarship award year
- Degree pursued and expected completion date
- Two letters of reference (at least one from teacher, academic counselor, coworker, or employer)
- Professional photograph (jpg, jpeg, or png format; max 10 MB)
- Up to 3 PDF files (max 10 MB each)

- Brief description of work experience, career goals, and future plans
- Extracurricular activities and community involvement
- Interest in serving in a rural healthcare setting in North Central KS
- Explanation of why you selected to work in healthcare and what strengths or personal experience you have
