Grants The Intermountain Community Care Foundation awards grants to organizations whose programs align with Intermountain Health’s mission, our national community health areas of focus, and priorities identified through our Community Health Needs Assessment. By supporting evidence-based programs and services that strengthen communities, the Foundation is dedicated to helping people live the healthiest lives possible. What We Fund We provide grants to organizations whose programs align with Intermountain Health’s mission, our national community health areas of focus, and priorities identified through our Community Health Needs Assessment. Healthcare Access GrantsSocial Drivers of Health GrantsChild & Family Mental Well-Being GrantsUtah Education Innovation Grants Utah Education Innovation Grants Intermountain Health is seeking Education Innovation programs that strengthen K–12 educational outcomes by improving access to quality education. Eligible programs should demonstrate innovation—either as early-stage pilots within their first three years or as proven, high-impact models being introduced into new regions where the approach is considered novel. Strategic investment areas include student and educator mental health and well-being, early childhood education, technology and Artificial Intelligence (AI), community-based youth support, teacher training and professional development, and initiatives that increase access to higher education and profession-based learning opportunities for students. Strong applications include collaboration and established relationships with educators and local schools. Grant funding should be considered a one-time investment intended to launch or establish a program with a clearly defined plan for long-term sustainability, rather than a source of ongoing financial support. All applications must include: A full and complete financial audit within the last two years. Submission must include the full audit, including any findings or letter from the auditing firm A 501c3 designation letter or have a comparable government designation Completed W9 An organization can apply for multiple Education Innovation grants to fund different projects. Each project requires a separate application.

Organizations funded for muti-year grants will not be eligible to apply again until after the final outcomes report has been reviewed by the board. Organizations can only have one active grant at a time.
