- Verify that certain requirements are met
4. Receive Application (if eligible)
- If eligibility requirements are met, organization will be invited to complete an Implantable Device Donation Application
- Complete and submit the application
5. Review Process
- CHI organization reviews all completed applications
- Approval decisions are made based on review
- Note: Application receipt does not guarantee device approval
