- Download the Flyer & Nomination Form from the CHI website

2. Complete Nomination Form
- Provide nominee information
- Describe why the individual deserves recognition
- Explain how they support deaf and hard of hearing children

3. Submit Nomination
- Follow instructions on the nomination form for submission
- Nominations can be made for students, professionals, relatives, or friends

4. Award Selection
- CHI reviews nominations
- Award winners are selected annually
- Winners are notified and invited to award ceremony
