The Impact Grant supports organizations addressing social drivers of health and community needs. The grant empowers organizations to identify barriers and provide immediate solutions to help people access essential services and support.

Focus Areas

The grant has supported initiatives addressing:

• Healthcare access and patient support services
• Transportation barriers - partnerships to ensure patients can access medical appointments
• Food insecurity - providing emergency food support and connecting patients to resources
• Maternal and child health - supporting expectant mothers and newborn care
• Emergency assistance - supporting vulnerable populations in crisis

Example Application

Cameron Health received an Impact Grant to address social drivers of health:

• Established partnerships with transportation providers (STAR Transportation, Northern Lakes, Dedicated Cabs, Taxi Boy) to ensure no patient misses clinical appointments due to lack of transportation
• Created food support partnerships with FYCO in Fremont for patients facing food insecurity
• Partnered with Iris Family Support Center to provide education and basic necessities (car seats) for newborn safety
• Provided emergency transportation home for Emergency Department patients without local support systems
