Cameron Health received an Impact Grant to address social drivers of health:
• Established partnerships with transportation providers (STAR Transportation, Northern Lakes, Dedicated Cabs, Taxi Boy) to ensure no patient misses clinical appointments due to lack of transportation
• Created food support partnerships with FYCO in Fremont for patients facing food insecurity
• Partnered with Iris Family Support Center to provide education and basic necessities (car seats) for newborn safety
• Provided emergency transportation home for Emergency Department patients without local support systems
