Overview

High Mountain Hay Fever Children's Health Fund

Geographic Scope: Custer County, Colorado

Focus Areas: Children's health (birth to 18 years), including but not limited to:

  • Accessibility of healthcare (medical and dental)

  • Mental health

  • After school and pre-school programs

  • Childhood obesity

  • Food insecurity

  • Immunization

  • Developmental screening

  • Child abuse and neglect

  • Smoking and tobacco use

  • Drug and alcohol abuse
  • Eligible Applicants: Nonprofit organizations and governmental programs in Custer County only. The Community Foundation does not make grants to individuals.

    Award Characteristics:

  • Small and large grant requests will be considered

  • Level of grant awards determined based on applications

  • Grants may be requested for all or a portion of project budget

  • Projects must be completed within 12 months of award date (extensions may be considered case-by-case)
  • Priority Considerations:

  • Applications requesting matching funds to support other grant requests from outside Custer County

  • Applications illustrating collaboration among multiple Custer County nonprofits

  • Applications supported by evidence-based programs

    1. Project Description (maximum 2 pages, PDF/DOC/DOCX):
    - Purpose, goals, and objectives
    - Expected impact of the project
    - Children's health need being addressed
    - How project effectiveness will be evaluated
    - If continuing beyond grant period: how sustainability will be accomplished

    2. Project Timeline (may be presented in table format):
    - Major activities
    - Participating groups/individuals/volunteers and their roles
    - Start date
    - Target completion date
    - Individual responsible

    3. Total Budget (detailed outline):
    - Budget requirements for the project
    - As much detail as possible

  • Application Guidelines - Your application should:

  • Illustrate the impact of the organization and its project on children's health in Custer County

  • Demonstrate a track record of developing new projects and successfully managing long-term projects

  • File one project per grant application

  • Demonstrate the administrative and fiscal strength and stability of the organization

  • Show how the project addresses an important problem or critical need in child health

  • Illustrate how completion of the project will be measured to demonstrate success

  • Outline potential problems and alternative strategies for the project

  • Demonstrate sustainability of the program

  • Provide a detailed budget
  • Post-Award Requirements:

  • Awardees must submit a final summary report of how grant funds were used and the impact the project had within the community
