Overview

HFC Caregiver Respite Program - Adult Day Center Respite Grant

Purpose: Cover the cost of professional Adult Day Center care so caregivers can take time for themselves while ensuring quality care for their loved one with dementia.

Award Details:

Type: 24 days of respite care at Adult Day Center

Duration: Must be used within 3 months of first date of care

Format: Professional Adult Day Center care only (NO CASH VALUE - cash awards cannot be issued in lieu of services)

Provider: Coordination through HFC's network

Geographic Scope:

United States and Canada

Puerto Rico and other U.S. territories are not currently eligible

Eligibility Requirements - Must answer YES to ALL:

Person being cared for has been professionally diagnosed with: Alzheimer's disease, frontotemporal dementia (FTD), Lewy body dementia (LBD), vascular dementia, mixed dementia, other dementias, or Parkinson's related dementia

Person being cared for currently lives at home with applicant

Applicant is facing financial and emotional hardships due to challenges of caregiving

Applicant resides in the United States or Canada

Reapplication Policy:

Only one application per person accepted quarterly (must wait at least 3 months between applications)

Maximum of 3 awards can be issued to same recipient within 18 months

Reapplying does not guarantee a new grant or same level of award as previously received

Important Notes:

If care cannot be provided by a licensed provider within 60 days of award, grant becomes void

Applicants are not guaranteed three grants or any specific level within 18-month period
