Overview

Heart Truth® Community Action Program

Overview: A grant program jointly managed by FNIH and the National Heart, Lung, and Blood Institute (NHLBI) that offers grants to empower community organizations to assist women in identifying personal risk factors for heart disease and making sustainable lifestyle behavior changes.

Focus Areas:

  • Heart disease prevention and health awareness

  • Community-level health interventions

  • The Heart Truth national awareness program messaging
  • Target Population: Women, especially those of color, low income, and/or living in rural areas

    Program Goals:

  • Empower community organizations to assist women to identify personal risk factors for heart disease

  • Support sustainable lifestyle behavior changes that support heart health

  • Lower risk for heart disease among target populations

  • Promote Heart Truth messages about the risk of heart disease in women

  • Evidence-based approaches required
  • Track Record: Since 2009, FNIH has awarded approximately $1.8 million to 37 organizations for creative community-level programs encouraging women to take action to lower their risk for heart disease.
