Purpose: Create projects that uplift health justice by engaging multiple partners to foster change for specific community-level health injustices.

Eligible Project Activities:

1. Address laws and policies that have historically created health inequities
2. Generate new laws and policies aimed at creating a more health-equitable future
3. Provide direct service addressing needs caused by health injustice

Required Elements:

• Commitment to engaging multiple partners
• Focus on fostering change for specific community-level health injustice
• Demonstrated community need

Background: Initiative launched March 2023 to address systemic health inequities in the region. Represents Partnership's role as catalyst, advocate, and collaborator.

Application Process

1. Request complete Request for Applications document
2. Complete application with required elements
3. Submit by deadline

Required Materials:

• Project description addressing health injustice
• Evidence of community need and injustice
• Multi-partner engagement plan
• How project addresses laws/policies OR creates new policies OR provides direct services
• Budget and timeline
• Organizational capacity
