Overview

Diabetes causes illness and death in the United States. It leads to high health care costs. People who use Federally Qualified Health Centers (FQHCs) and other safety-net clinics face greater risks. Many community-clinical linkage (CCL) strategies show benefits when used alone. Using multiple strategies together may improve diabetes prevention and management in primary care.

This NOFO aims to find what helps or blocks the use of coordinated CCL strategies. Researchers will study real-world FHQC clinics. Practical evidence will guide best practices for diabetes prevention and management.
