Grant Application Process

Submission Format:

Required Application Content - General Information:
1. Legal name of organization
2. Address, phone, website
3. Name of executive director/director/head
4. Description of the organization (50 words or less)
5. Contact person and title; email, phone(s), fax
6. Organization's operating budget for current fiscal year
7. Disclosure of any legal proceedings involving organization or board members/officers in last 5 years

Required Application Content - Proposal Summary:
1. Summary of the proposal (50 words or less)
2. Funding amount requested from Last Call Foundation
3. Total project budget
4. Anticipated funding sources (LCF and others)
5. Amount and dates of previous support from Last Call Foundation, if any

Required Application Content - Proposal Narrative:
1. Statement of mission for the organization
2. History and key achievements
3. Size and structure of the organization
4. List board members and their qualifications/affiliations
5. Number of staff and volunteers expected to be engaged in proposal
6. Description of proposed work:
- Statement of problem or need being addressed
- Target population
7. Project goals and objectives with milestones and reasonable projections
8. Expected deliverables and dates
9. Anticipated outcomes or benefits
10. Budget allocation: How funds will be used to complete project (include line-item expenses)
11. Methodology: Brief outline of phases and associated budgets, with timelines; allocation of staff
12. Evaluation: How will success be measured?

Required Attachments:
1. IRS letter confirming tax-exempt status (501(c)(3) or 509(a) if applicable)
2. Most recent independent audit or account review and/or year-to-date financial statement for current fiscal year
3. List of other organizations, foundations and funding sources being approached (if applicable) with:
- Dollar amounts
- Status (committed, pending, or anticipated)
- Contact information
4. Letters of recommendation in support of proposed funding from:
- Fire service officials
- Medical professionals
- Mental health officials
- Safety officials
- Other relevant experts
