NOTE: All applications submitted here will be reviewed in the final quarter of the calendar year. Any requests submitted after September 1 of the current calendar year will not be considered until the next calendar year. Funding Information The Foundation is committed to funding projects that directly align with our focus areas: Access to Health Care and Children’s Health. However, when weighing the merits of any proposal, we will also consider: The potential impact of the proposed activity. The utilization of existing evidence and best practices.The likelihood of a project to lead to actionable policy recommendations or systemic change.Is the project equitable and have the potential to reduce health disparities?The ability of a project to drastically increase the knowledge base of consumers, advocates, and influencers. The potential for growth of a project following the initial investment or grant. We cannot promise that any grants will be made as it is dependent on what funds are available and what applications meet our priorities. The Foundation for a Healthy Kentucky will review and select applications submitted by eligible organizations, and the selected organizations will be notified of the award via email. Please note: Projects do not require a cost sharing or funding match. Grants will be paid in full upon signing of grantee contracts. All funded activities must take place within the dates agreed upon in the contract. The Foundation may request meetings with the grantee, convene a cohort with similar projects, or conduct site visits in order to provide technical assistance and evaluation.

Applicant is located in the state of Kentucky.  Applicant provides services in the state of Kentucky without discrimination. Applicant’s commitment to supporting diversity, equity, and inclusion in their community as reflected via representation in their board of directors, staff, and leadership. Projects must include evidence-based or evidence-informed approaches.
