Extremity Scholarship
Overview: The Limb Preservation Foundation offers a four-year scholarship for outstanding achievement to help with higher education costs for individuals with limb difference or limb loss.
Geographic Scope: Rocky Mountain Region – Arizona, Colorado, Idaho, Kansas, Montana, Nebraska, New Mexico, Utah, and Wyoming

Application Instructions
1. Extremity Scholarship Application - Use this application if the patient is applying for a first-time scholarship
2. Extremity Scholarship Reapplication - Use this application if the patient has previously submitted a scholarship application with hopes to renew
3. Submit through the online form on the scholarships page
4. Provide accurate contact information
5. Applications are reviewed on a rolling basis
6. Applicants will be notified within two days of the committee's review
