How to Apply for Evergreen Program

1. Complete Veteran Application Form:
   • Access at cellphonesforsoldiers.com
   • Select "Evergreen Program" as preference
   • Provide required information
2. Application Review:
   • Application reviewed for approval
   • Once approved, veteran receives link to complete registration within Evergreen Program system
3. Complete Evergreen System Registration:
   • Follow link provided
   • Complete registration in Evergreen system
4. Choose Your Service Tier:
   • Select appropriate plan ($25, $60, or $120/month)
   • Bring your own device or select new device option
   • Decide whether to add family member
