Equipment Loan Program

Durable Medical Equipment (DME) loan program providing short-term and long-term equipment loans at no cost to individuals living with Duchenne Muscular Dystrophy (DMD) and Spinal Muscular Atrophy (SMA).

Geographic Scope: New England

Sponsor: Rehabilitation Equipment Associates (REQ)

Objectives:

• Loan out durable medical equipment to people with DMD and SMA in New England at no cost
• Collaborate with other nonprofits to meet equipment needs
• Help find equipment not currently available in the rotation

How It Works:

• Short-term and long-term equipment loans available
• Equipment rotation system
• If needed equipment is not available, the program will help locate it

Eligibility:

• Living in New England
• Diagnosed with DMD or SMA

How to Request Equipment:

• Fill out the equipment request form available on the website
• Someone will contact the applicant within 1-3 business days

How to Donate Equipment:

• Email jb@jbskeys.org with "Equipment Donation" in the subject line
• Someone will respond within 1-3 business days

How to Request Equipment

1. Access the equipment request form on the website
2. Fill out the form completely
3. Submit the form
4. Someone will contact you within 1-3 business days with equipment availability and next steps

How to Donate Equipment

1. Email jb@jbskeys.org with "Equipment Donation" in the subject line
2. Include details about the equipment you wish to donate
3. Someone will contact you within 1-3 business days to arrange pickup or delivery
