Overview
Czech and Slovak Sokol Minnesota Youth Leadership Award
Purpose: To encourage and recognize outstanding participation of youth members and foster a future generation of members and leaders for Czech and Slovak Sokol Minnesota organization.
Award: Up to three Sokol Minnesota youths will receive the main award with a $500 cash prize ($400 from Sokol Minnesota, plus $100 from Honorary Czech Consul M. L. Kucera). A separate award with a $100 prize is given for the Best Essay, donated by Louise Wessinger.
- Grade Level: Must be in junior or senior year of high school
- Membership: Applicant and parent(s) must be paid members of Sokol Minnesota for at least the current and preceding calendar year
- Activity Participation: Must actively participate in one or more Sokol Minnesota sponsored youth activities, such as: - Taneční Teen dancers - Sokol gymnastics - Czech and Slovak Children's Cultural Day Camp - Sokol Minnesota Youth Link - Czech & Slovak School Twin Cities activities
- Volunteering: Must be a regular volunteer at Sokol Minnesota sponsored events
- Prior Awards: Applicants can apply multiple times but may receive the award only once

How to Apply
Application Requirements and Process
Application Period: April 1 – July 1 (annually)
Required Materials:
1. Youth Leadership Award application form (outlining activity participation)
2. Essay: 300-word maximum essay about how participation in Sokol Minnesota activities has impacted applicant's life
3. Participation Verification Form: Signed by an adult director of youth activity
4. Letter of Recommendation: From an adult director
5. Volunteer Verification Forms: Signed by adult leaders of Sokol Minnesota events where applicant volunteered
Submission Process:
1. Complete all application forms (available for download on website)
2. Send all completed forms to: YLA@sokolmn.org
3. Applicants will receive confirmation within one week that forms were received
4. Award recipients announced by September 1
5. Awards presented at annual Sokol Minnesota Czech and Slovak Festival (typically late September)
