Community Need Fund

Geographic Scope: Ten-town service area including Bethlehem, Cheshire, Middlebury, Prospect, Southbury, Thomaston, Waterbury, Watertown, Wolcott, and Woodbury, Connecticut

Purpose: The United Way of Greater Waterbury aims to address the ongoing and emerging needs of the community by investing in programs aligned with adopted Impact Area strategies.

Eligibility:

• 501(c)(3) organizations in good standing
• Must provide services in the United Way of Greater Waterbury ten-town service area

Impact Areas:

• Education
• Financial Stability
• Basic Needs

Requirements for Grant Applications:

1. Alignment with United Way Outcomes and Strategies
2. Statement of Purpose and Use of Funding Intent:
   • Intent of program proposed
   • Community need defined and documented
   • Target population identified
   • Outcomes to be achieved through program for individuals and the community
   • Amount requested and specific use of United Way Funds
   • Grant period for the request
3. Data Collection Plan:
   • What will be reported back to United Way and how data will be collected
   • Measurement tool to be used
4. Agency Budget:
   • Approved budget
   • Audit and IRS 990
5. Program Budget:
   • Approved budget
   • Include any leveraged funds or in-kind services planned

Application Requirements

Required Materials:

1. Statement of alignment with United Way outcomes and strategies
2. Statement of Purpose including:
   • Program intent and proposed activities
   • Documentation of community need
   • Identification of target population
   • Specific outcomes to be achieved (individual and community level)
   • Specific amount requested and detailed use of United Way funds
   • Grant period/timeline
3. Data Collection Plan:
   • Reporting metrics and data collection methodology
   • Measurement tools
4. Agency Budget:
   • Current approved budget
   • Most recent audit
   • Most recent IRS Form 990
5. Program Budget:
   • Approved budget for proposed program
   • Documentation of any leveraged funds or in-kind services
