Community-Led Momentum Grants

The Community-Led Momentum Grants, funded by the Advancing a Healthier Wisconsin Endowment, offer up to $250,000 for community-driven health initiatives in Wisconsin. These grants aim to support innovative strategies and programs that enhance the health workforce and promote health equity. Funding Summary
AHW’s Community-Led Momentum Grants support community-driven efforts to implement new public and community health strategies, develop innovative programs and resources to advance Wisconsin’s current or future health workforce, and carry out novel efforts to inform policy and practice to advance health and health equity in Wisconsin. Successful projects will engage a collaborative team of community and MCW partners to demonstrate impact on the community-identified health need during the project period. Applicants may request up to $250,000 for a 24 month period, beginning July 1, 2026.
