Overview

Funding Summary AHW’s Community-Led Momentum Grants support community-driven efforts to implement new public and community health strategies, develop innovative programs and resources to advance Wisconsin’s current or future health workforce, and carry out novel efforts to inform policy and practice to advance health and health equity in Wisconsin. Successful projects will engage a collaborative team of community and MCW partners to demonstrate impact on the community-identified health need during the project period. Applicants may request up to $250,000 for a 24 month period, beginning July 1, 2026.
