Community Health Grants

The Clarksville-Montgomery County Community Health Foundation (TN) funds 501(c)(3) organizations for one-year, health-impact projects in its six-county service area, with two deadlines each year: May 1 and November 1. Applicants must first discuss proposals with the Grants Manager and submit a 501(c)(3) letter, itemized budget, and measurable outcomes report; site visits are required. Individual awards historically range from a few thousand to several hundred thousand dollars. Geographic scope: Montgomery/Cheatham/Dickson/Houston/Robertson/Stewart counties, TN + Ft. Campbell.

Apply via the CMC Community Health Foundation after a required proposal discussion with the Grants Manager. Funds 501(c)(3) organizations for one-year projects that address at least one driver of health (health behaviors, clinical care, built environment, social/economic factors) benefiting Montgomery (incl. Ft. Campbell), Cheatham, Dickson, Houston, Robertson, and Stewart counties (TN). Two windows: applications due May 1 and Nov 1 each year (decisions at the July and January board meetings). Requires 501(c)(3) letter, itemized budget, outcomes report, board list with a local member, and letters of support/MOAs.
