Geographic Scope: Clarksville-Montgomery County community area and surrounding areas

Focus Areas:

• Tobacco use prevention and control
• Smoking cessation programs
• Evidence-based tobacco use intervention projects
• Underage tobacco use prevention
• Smoking as a health behavior intervention

Background: Given the importance of tobacco use as an influence on community health and the lack of prior funding requests in this area, the Foundation is prioritizing tobacco use projects by making funds available. Research shows that smoking as a behavior is the health factor with the highest influence on community health.

Funding Priority: The Foundation has specifically designated funding availability for evidence-based projects aimed at eliminating smoking as an adverse health behavior in the Clarksville-Montgomery County community.

Project Requirements:

• Must be evidence-based approaches
• Focused on controlling smoking prevalence
• Aimed at eliminating smoking as an adverse health behavior

Application Process

1. Contact the Grants Manager to schedule a meeting to discuss your tobacco control proposal at least 30 days prior to the application deadline
2. If eligible, you will receive the CMC Community Health Foundation Grant Application and Outputs and Outcomes Expectations Report
3. Submit your completed application with all required materials by the deadline

• Application Window #1: January 1st - May 1st
• Application Window #2: July 1st - November 1st
