Application Process

1. Complete the Community Grant Application - Available at ocysf.org/grant-application/
- Application must be completed in detail
- Be as specific as possible
- Incomplete applications will not be approved

2. Required Information to Provide:
- Name of Applicant Organization
- Organization's purpose and mission
- Amount of funding requested (1-999,999)
- Detailed description of funds intended use
- Target population served
- Organization address and contact information
- Phone and email
- Non-profit incorporation status (yes/no)
- Date of incorporation (MM/DD/YYYY format)
- Federal Identification Number (9 digits)
- State Identification Number (9 digits)
- Organization social media accounts (optional): Instagram, Twitter/X, Facebook, TikTok

3. Program Contact Details:
- Program Contact Name and Title
- Phone and Email
- Confirm if contact address differs from organization address

4. Financial Information:
- Actual Fiscal Year Revenue 2021-2022
- Actual Fiscal Year Revenue 2022-2023
- Actual Fiscal Year Revenue 2023-2024
- Indicate if previously received OCYSF funds and what years

5. Long-Form Essay Questions (Required):
- Summary of Organization Goals
- Describe the organization's history and experience in providing the proposed services
- Briefly explain the organization's experience utilizing volunteers and how volunteers will be utilized in the proposed program
- How does the organization generate other funding (fees, donations, event revenue, planned giving)?
- List the name and title of each Board of Directors member

6. Board Confirmation:
- Must be signed/confirmed by the Chairman or President of the Board of Directors
- Confirm that the Board of Directors has full knowledge and approval of the grant application
- Confirm that funds will be used for program/services as described

7. Submit:
- Form includes CAPTCHA validation
- "Save & Continue" button to submit
