Mission Alignment: OCYSF funds sports programs and activities for at-risk or disadvantaged youth in Orange County. Applicants must describe how their requested funds align with OCYSF's mission statement.

Target Populations

Applicants must identify the target population their program serves. Programs should serve economically disadvantaged youth and other at-risk populations in Orange County.

Eligibility Requirements

• Organization Status: Must be a 501(c)(3) non-profit organization incorporated in California
• Required Information:
  • Federal Identification Number
  • State Identification Number
  • Board of Directors list with names and titles
  • Actual fiscal year revenue for 2021-2022, 2022-2023, and 2023-2024
  • Program Contact information
  • Organization mission and history

Reporting Requirements

• Organizations receiving funding must submit a follow-up report 6-12 months after receiving funds
• Report should provide an update on how funds were used
• Failure to report could influence future grant decisions

   • Detailed description of funds intended use

   • Non-profit incorporation status (yes/no)
   • Date of incorporation (MM/DD/YYYY format)
   • Federal Identification Number (9 digits)
   • State Identification Number (9 digits)
   • Organization social media accounts (optional): Instagram, Twitter/X, Facebook, TikTok

   • Confirm if contact address differs from organization address

   • Actual Fiscal Year Revenue 2021-2022

5. Long-Form Essay Questions (Required):
   • Summary of Organization Goals
   • Describe the organization's history and experience in providing the proposed services
   • Briefly explain the organization's experience utilizing volunteers and how volunteers will be utilized in the proposed program
   • How does the organization generate other funding (fees, donations, event revenue, planned giving)?
   • List the name and title of each Board of Directors member
6. Board Confirmation:
   • Must be signed/confirmed by the Chairman or President of the Board of Directors
   • Confirm that the Board of Directors has full knowledge and approval of the grant application
   • Confirm that funds will be used for program/services as described
