Wisconsin Medical Society Foundation Community Grant

Mission: To advance the health of the people of Wisconsin by supporting medical education and health initiatives.

Geographic Scope: Wisconsin

Program Focus: Physician-led, community-based or statewide programs to improve health through education and outreach. High-impact, high visibility programs are prioritized.

Funding Preferences

Primary Focus Areas:

  • Health literacy

  • Health access and equity

  • Health awareness

  • Adverse childhood experiences reduction or mitigation

  • Mental, behavioral and addiction health access and prevention services

  • Disease prevention (including school or community gardens)

  • Medical outcomes research and/or interpretation and dissemination of research

  • Other evidence-based health topics or innovative pilot projects
  • Limited Funding Available For:

  • Physician education

  • Medical outcomes research and/or interpretation and dissemination of research

  • Initiative Preferences:

  • Promote changes to modifiable factors affecting health

  • New or innovative projects

  • Focus on prevention

  • Well-defined and reasonable to accomplish within grant period

  • Involve or are supported by a physician member of the Wisconsin Medical Society
  • Overarching Goal: Improve health equity in Wisconsin

    Eligibility

    • Must be physician-championed or physician-supported
    • Projects must be evidence-based or new/innovative pilots
    • Must have measurable outcomes

    What the Foundation Does NOT Fund

    • Unrestricted general operating expenses
    • Use and payment for services of a fiscal agent
    • Endowment funds
    • Religious organizations for religious purposes
    • Fundraising activities or events (annual fund drives, benefit tickets)
    • Umbrella funding organizations that intend to distribute funds at their own discretion
    • Political lobbying or legislative activities
    • Individuals
    • Capital expenditures (building repairs, etc.)

    Application Timeline

    • August 1: Foundation staff send application invitations to organizations recommended by physicians; application opens
    • October 15: Grant applications due
    • November: Grant recommendations made
    • December: Funding decisions approved by Board; notifications sent second week of December

    Required Application Materials

    • Online grant application form
    • Budget document (downloadable)
    • Certification document (downloadable)
    • Identification and role of any subcontractors with associated budget
    • Hourly rates for all subcontractors
    • Disclosure of all funding sources for the project

    Post-Award Requirements

    Upon Grant Award:

  • Sign a letter of agreement/memorandum of agreement

  • Acknowledge "Wisconsin Medical Society Foundation" in printed materials, press releases, and published materials as appropriate; provide copies to Foundation
  • Grant Agreement Stipulations:

  • Communicate significant changes in scope, time, or budget to Grants Committee through Program Coordinator as soon as known

  • Disclose all funding sources for the project fully

  • Return any funding not specifically used for this project to the Foundation at end of grant timeline

  • File a final report noting precise outcomes and costs (submit via online final report form)
  • Important Notes

    • Foundation Board of Trustees or designees are only agents who may legally commit Foundation to expenditures
    • No costs may be reimbursed before receipt of fully executed letter of agreement
    • Applicant may subcontract with other entities but must deal directly with them and be responsible for performance and payment
